You are invited to participate in this volunteer survey which will help Kuumbwa Jazz, a Santa Cruz based nonprofit arts organization, learn about continuing to offer the best in arts and culture to enrich our communities. We would like to learn how, when, where and why music is important in your life.

Each completed survey will be entered into a drawing to win a $50 gift certificate from a terrific local restaurant, plus complimentary tickets to a Kuumbwa Jazz presented concert!

Kuumbwa Jazz has grown from small beginnings in Santa Cruz to one of the most highly respected Jazz-presenting institutions in the country, attracting artists of national and international renown. Kuumbwa Jazz presents more than 120 concerts per year in Santa Cruz County. Since 1975, this nonprofit has presented the full spectrum of Jazz, preserving the history and legacy of this unique art form.
