Maine Provider Database - Data Collection Tool
 

1. Introduction

 
Primary care practices across Maine have made significant efforts over the past several years to improve the quality of care they deliver, including efforts to measure and report on quality. Recognizing that work, Quality Counts, the Maine Health Management Coalition and the Maine Quality Forum are jointly launching an effort to create the "Maine Provider Database" to collect and maintain accurate data about practices that can be used in our public reporting efforts.

We plan to use this information in efforts to publicly recognize excellent performance such as the MHMC's "Pathways to Excellence" (PTE) program. In addition to using this information to make sure we accurately list and describe practices, we are would also like to use this effort to collect data about practices that patients have told us is important to them - e.g. information on practice accessibility and office hours, available online services, and use of EMRs and disease registries.

We plan to share this information with patients and consumers on the updated PTE website, which we anticipated launching this fall.

In order to collect accurate information, we are asking one person from your practice site (generally the practice manager, medical director, or other practice leader) to complete this online data collection tool by September 30, 2010. This information should be completed for each physical practice SITE (i.e. if you have a group with three sites, please complete for EACH site).

The tool consists of 15 sections and is estimated to take approximately 20-30 minutes to complete. It includes space to enter information for up to 20 clinicians; if you have more than 20 primary care clinicians practicing at this site, please contact Stephanie Pratt to provide the remaining information. After initial completion, practices can go in anytime to update their data.
