We are delighted to have your participation in the MOUSE network! Your answers will help us deliver the most appropriate services to your MOUSE Squad (or, in Chicago, CPS Tech Crew). Please answer the questions in this survey to the best of your ability. We understand that those of you in the Chicago area use the name Tech Crew. Hhowever, for the purposes of this survey we will use the name MOUSE Squad.
The survey is completely confidential; that means that we will not share your name or your individual information with anyone. The survey should take less than 30 minutes to complete.
