We would like your feedback regarding the Mix publication and your experiences at natural food co-ops in the Twin Cities area. This is not a sales effort of any kind. Please be assured that your responses will be kept strictly confidential and will be used by the Mix/co-ops in aggregate form only. Your individual responses will not be revealed.

This survey should take approximately 10 minutes to complete. If you are interrupted or cannot complete the survey, you can restart the survey from the beginning by returning to this website. When you complete the survey, you will have an opportunity to enter our drawing for one of three $50 gift cards redeemable at the Twin Cities natural food co-ops.
