Every 4-H Shooting Sports instructor/leader needs to submit the Club/Local Level 4-H Shooting Sports Instructor Report each year in order to remain on the mailing list and instructor list as an active 4-H shooting sports instructor. Even if the instructor does not teaching throughout the year they need to submit a report with the box checked “no teaching to report this year.”

This report is for the September 2009 to August 2010 Programing year.

Please be careful when filling out this form. There are two question that should only be answered by one of the club leaders, and the information is for the entire club.

2. Did you teach or assist with classes, offered trainings or conducted any shooting sports programs in the 2009 / 2010 program year? This can include helping with a fair booth, working with clubs, camps or other 4-H shooting sports related activities .

County Fairs... this section is for hands on learning experiences at your county fairs, not individual presentations or displays. If you are not the lead instructor /club leader, please skip this question.Please only one response per club for questions 5 and 6.

7. Clubs... How many youth and how many club hours did you spend instructing youth in the following areas. This can be hands on teaching or instruction for your curriculm. If you did not teach, please enter "0" in the top field. Please round all answers up to the next hour. Please do not enter fractions or percents (example 4.5).

8. Camps... How many youth and how many hours did you spend per child instructing youth in the following areas ( for example, if you taught 100 youth, and each youth got 5 hours of instruction, you would indicate 100 youth and 5 hours)

9. Shooting Sports Clinics... (instruction with youth who were not members of your club)

If you do not submit a report each year you will be dropped from the 4 H shooting sports instructor list! The purpose of the report is both to have accurate data regarding number of youth impacted by the 4 H Shooting Sports program and to help states manage an active list of 4-H Shooting Sports instructors.
