Monthly Staff Excellence Awards

We have launched a new Monthly Staff Excellence Award scheme to reward those members of Kingston Hospital Staff who have gone above and beyond in their day to day job.
Each month the prize will be a £25 Amazon gift card.

We are inviting patients to nominate any individual member of staff who they think is deserving. All you need to do is complete this nomination form and click submit.

How do I nominate?

Decide which of the four headings you want to nominate under; Caring, Safe, Responsible or Value each other. Then tick the behaviours listed underneath which you think applies to your nominee.

You will also need to write a short supporting paragraph no more than 100 words and be happy for it to be used as part of the citation read out by the Chief Executive, when the award is presented.

Remember to keep your nomination secret as we want to surprise the winner every month!
