This ‘survey monkey’ of UU congregations is designed to assist the St Lawrence District in assessing what might be most helpful to our congregations and leadership in moving towards environmental sustainability.
Thank you for taking time to answer the 18 questions.
*It is fine to have more than one person fill out the survey from your congregation. We welcome the perspectives of ministers, lay leaders and interested members and friends.

1. What is the name of your UU congregation?

2. Please check your role in the congregation.

3. Does your congregation have a group responsible for environmental issues? Please provide the name of that group(s) and contact information for the leader(s).

4. What activities do you feel especially good about that your congregation has sponsored to address environmental issues?

5. Check the environmental issues that are of most interest and concern in your congregation? Has your congregation worked on any of these issues or plans to in the immediate future?

6. Please check the relationship of your congregation to the UUA Green Sanctuary and or UU Ministry for the Earth.
