To complete the survey use the appropriate number with the following scales:

Column 1 How Important: 1 = Completely Unimportant, 2 = Unimportant, 3 = Important, 4 = Very Important

Column 2 How Satisfied: 1 = Completely Unsatisfied, 2 = Unsatisfied, 3 = Satisfied, 4 = Very Satisfied
