INFORMATION SHEET FOR PARTICIPATION IN RESEARCH STUDY
THE PERCEIVED EFFICACY OF HOSPITALS’ PRENATAL BREASTFEEDING CLASS AND ITS EFFECT ON BREASTFEEDING INITIATION AND DURATION
You are being asked to participate in a research study being conducted by Lisa Frantze and Amanda Samson, graduate students at DePaul University, as a requirement to obtain their Masters degrees. This research is being supervised by a faculty advisor, Dr. Ron Graf. We are asking you because we are trying to learn more about the efficacy of health institutions’ prenatal breastfeeding course on initiation and duration of breastfeeding. This study will take about 15 minutes of your time. Survey responses will be completely anonymous. If you agree to be in this study, you will be asked to fill out a questionnaire. The questionnaire will include questions about prenatal breastfeeding courses. You will also be asked questions about you such as age, number of children, and education level. You can choose not to participate. There will be no negative consequences if you decide not to participate or change your mind later.
