If you run more than one carescheme, please fill in a survery for each one, making it clear in the Provision Name section which it is. For example, if you run a Before School Club, an After School Club and a Holiday Scheme, you should fill in three surveys.

If you run a provision that is not open during Headcount week please choose a recent typical week and base your answers on that.
