TOURNAMENT INFORMATION

Format: Scramble
Where: Stonehenge Golf & Country Club, Warsaw
When: Friday, September 9, 2011 (Rain Date ~ 9/12/11)
Tournament results, raffle and prizes will be awarded after PM play at the Stonehenge Clubhouse

Entry Fee: $500 per Team ($125 for individual player)
Entry includes:
~ Golf cart
~ Favors
~ Hole-in-One contests
~ Tournament Prizes
~ Breakfast for AM players
~ Lunch for all players
~ Hors d'oeuvres for PM players

You may also participate in any or all of the following:
~ $5000 Putting Contest - $20 per team of 2 players (Unlimited Entry)
~ Mulligans for sale - $10 each
~ Par 3 contests - $5 each
~ Team skin game - $20 per golf team
Remember, the reason we are here today...to raise money for the Kosciusko County Cancer Care Fund!

Liability: Players play at their own risk. The KCCCF Directors, Committee Members or volunteers assume no responsibility or liability for any accident or injury incurred during the tournament.

This registration is for teams and individuals. If you are registering a team, you will need the names of your team members.
