This form is due within 60 days of project completion. As a grant recipient you can answer and submit the majority of the information that we require by completing this form. Additional information that you need to mail or email includes your budget summary and an example of your use of the credit line. These items are explained as you complete this final report. * 8. Please type the number of people affected by your proposal in the spots below. If you have trouble answering this question you need to do a better job of estimating or counting in the future. It is important to track your impact as you measure outcomes. We require the totals only. You will need to determine whether an adult is actually participating or is in the role of audience member. List individuals one time only. These numbers are very important as we talk to legislators and secure future funding.

* 9. Did the project differ in terms of the main description in your grant proposal or in your budget? Here are some questions to ask: Did we shuffle money from one category to another? Did a presenter cancel so we needed to get approval for a replacement? Did we change our play title to another play after further review? There are many things that happen and this is your chance to let us know. Remember that you should ask formally for large changes to your grant request ahead of time.

You can submit your own spreadsheet just remember to show both the numbers that you submitted with your application and your actual numbers at the close of your grant activity/proposal. Your budget summary must include a typed name, title and official signature from your project director and authorizing official, and fiscal sponsor/agent if applicable.
