The following questions are to help you to let us know about the difference that having access to childcare for your 2 year child has made.

This includes any impacts on your child and the family.

1. What is the name of the Daycare your child attended?

2. Were you satisfied with the service you received from your Area Family Information Co-ordinator?
(Was he/she:Caring? Clear? Do things within timescale stated?)

3. Is your child still attending the free nursery place for 2 year olds?

4. How long has your child been attending a free nursery / pre-school place?

5. How many hours per week does your child attend the nursery / pre-school?

6. Was the nursery / pre-school able to offer the hours you wanted?

7. Since your child started at the nursery / pre-school, have you received any family support from your local Children’s Centre?
