INFORMED CONSENT
Project Title: Influence of Life Stress on Driving Behaviors

Invitation to Participate: You are invited to take part in a research study through the University of Northern Iowa. This study is conducted by a master’s level graduate student at the University of Northern Iowa. The University requires that you give your agreement to participate in this project. The preceding information is to help you make an informed decision to either participate or not participate in this study.

Nature and Purpose: The purpose of this study is to establish a moderator relationship between life stress, driving aggression, and motor vehicle accidents.

Explanation of Procedures: You will be asked to complete three surveys; The Life Experiences Survey created by Sarason, Johnson, and Siegel (1978) consisting of 57 items, The Driving Survey created by Deffenbacher, Deffenbacher, Lynch, and Richards (2003) consisting of 38 items that will assess your driving behaviors, and the Marlowe-Crowne Social Desirability Scale (1960) consisting of 33 true/false questions. All surveys are standardized measures and have been used in many other studies. After completion of those surveys, you will be asked to complete some items regarding your demographic characteristics. The entire study should take approximately a half hour to an hour to complete. There will not be any use of audio or video recording for this study. The data at the end of the study will be coded and put into a database that will be stored in a protected external hard drive.

Discomfort and Risks: The risks of you participating are minimal and those risks are similar to what you or another person experiences in everyday life.

Benefits and Compensation: You will not be compensated for your time to complete this study. Participation of this research study is voluntary and you may withdraw or refuse to participate in this study.
2. Confidentiality: Information obtained during this study will not need any direct identifiable information or IP numbers and everything will be kept confidential. Your confidentiality will be maintained to the degree permitted by the technology used. Specifically, no guarantees can be made regarding the interception of data sent via the Internet by any third parties. Data will be anonymously to a secure website. The summarized findings, with no direct identifying information, may be published in an academic journal or presented at a conference. The Survey Monkey website has SSL encryption, and an encrypted URL will be sent to the UNI employees. The link and survey pages will be encrypted during transmission from the primary investigator website account to the UNI employees and then back into the primary investigator’s website account on Survey Monkey.

Right to Refuse or Withdraw: You as the participant are free to withdraw and have the right to refuse to participate in this study because this study is on a voluntary basis. If at any time you decide to withdraw or refuse to participate, you will not be penalized for your participation.
