Introduction
The purpose of this survey is to gather complete and accurate contact information for your school so that the Office of School Improvement can communicate effectively with you during the 2012-2013 school year. This survey is designed for Title I schools that are required to create a school improvement plan due to not meeting one or more federal annual measurable objective (AMO).
