Lima Park Survey

About this survey...

Action Communities for Health, Innovation, and EnVironmental changE (ACHIEVE) is a collaboration between the Centers for Disease Control and Prevention (CDC), National Association of City and County Health Officials, and other selected national organizations. It is a movement to create healthier places to live, work and play.

Through ACHIEVE, the Livingston County Department of Health in partnership with community partners, is promoting health through such improvements as safe locations for physical activity and increased access to healthy food options such as fruits, vegetables and whole grains at schools, worksites, places of worship and in the general community, among others.

To increase physical activity opportunities, the Town of Lima is working to increase the community’s access to parks, shared-use paths and trails or open spaces by expanding the Mark Tubbs Park. Please respond by May 7, 2012. Thank you for completing this survey.
