1. Introduction
RMC Research Corporation has contracted with Louisiana State University to conduct an evaluation of the Louisiana Math and Science Teacher Institute (LaMSTI) Partnership. The purpose of this study is to document activities undertaken as part of the project and their impact. The results of this evaluation project are expected to provide valuable information at the local, state, and national levels about effective professional development activities for secondary science teachers. In addition, this information will enable participating schools to ensure that their programs are effective and that they have positive instructional practice, and other topics related to science.
As part of your agreement with the LaMSTI partnership, you agreed to participate in related research and evaluation activities. The information you provide as part of these activities will be essential to demonstrate its impact and to identify areas for improvement. When answering the questions, please keep in mind that every effort should be made to answer each question honestly and to the best of your ability. We will make every effort to keep the information we collect confidential, and you will not be identified in any report. The survey should take about 30 minutes to complete.

I (or a member of the evaluation team) will be happy to answer any questions you may have. If you have any questions about your rights as a research participant that have not been answered by the investigator, or to report any concerns about the study, you may contact the Institutional Review Board at Louisiana State University at (225) 578-5833.
