ISMP is conducting a short survey on health professionals’ viewpoints regarding topics of interest that have surfaced in response to the national outbreak of meningitis from contaminated compounded sterile preparations (CSPs). In particular, we are interested in learning more about disclosure regarding administration of high-risk CSPs and training and monitoring of compliance with USP Chapter <797>. This survey will remain open until December 14, 2012. We sincerely appreciate your input on these topics!

*Definition of high-risk CSPs: Products where non-sterile ingredients or a non-sterile device is involved in the preparation, which requires terminal sterilization before use. High-risk CSPs do not include simple mixtures of sterile ingredients, such as reconstituted antibiotic solutions or an insulin infusion.
