Public libraries no longer use this report form. Your ILL statistics for FY2013 will be reported on the Annual Report and Application for State Aid that public libraries complete each fall.

Instructions
Include any ILL transactions in which your library requests material from, or supplies material to, another library upon request of a user. This includes, but is not limited to, requests made through your circulation system; i.e.,"system holds" or "transit holds", reQuest ILLs, OCLC ILLs, and offline requests via phone, fax, e-mail or mail.
Borrowing Requests FY2013: Material your library requests from other libraries (use numbers only, no commas or spaces)
Lending Requests FY2013: Material your library supplies to other libraries (use numbers only, no commas or spaces)
