This survey is being conducted by the California Immunization Coalition in collaboration with the California Department of Public Health Immunization Branch in order to assess attitudes, preferences, and knowledge of new HPV vaccination indications. Your response with help inform us to develop appropriate messaging for health care providers, patients, and parents. We appreciate your time!
