Hospice Toronto is engaging in a disability outreach Initiative entitled “Transitions”.

We hope that Transitions will provide us with more effective ways of serving those with disabilities and their caregivers in our community who are facing end of life care needs. We hope to learn how we can help and what we need to know to better engage our diverse community.

For these purposes “end of life” is defined as being when life expectancy is no more than two years.

We know that people with disabilities do not typically access end of life care and we would like to know, from them, why.

People with disabilities, along with the mainstream population, are living longer. Informal caregivers and families do over 80% of care giving. There is a growing increase and focus on care giving throughout the country.

Your assistance is respectfully requested. Please take a few minutes to complete the short survey following. Your privacy is assured and we request that you respond only to those questions with which you feel comfortable.
