Quick Online Survey – Your organization’s feedback is important!
The Tampa Bay Partnership Regional Research & Education Foundation has launched a new initiative, ONE BAY: Healthy Communities. The initial goal is to produce a report that documents the health status of the Tampa Bay Community and the current initiatives throughout the region devoted to community health improvement. The report will serve as a mechanism to get our community’s leaders discussing the findings and working together to prioritize an action plan for moving forward.
We have made substantial progress in our data-gathering effort and are now seeking your assistance. On the next few pages is an online survey seeking information about your employees’ health and your organizational programs related to employee and community health.
We are hoping to get a high response rate in order to get an understanding of our corporate community. We’ve designed the questions to be fairly general so that the time required for completion is short. Take a look at the survey and if you are not comfortable answering the questions, please forward to the individual in your organization who can respond.
