New York State DOH OMH-HDP Health Literacy Survey

The NYSDOH Office of Minority Health and Health Disparities Prevention (OMH-HDP) has been leading health literacy efforts throughout the Department. According to the Calgary Charter on Health Literacy, health literacy is the use of a wide range of skills that improve the ability of people to act on information in order to live healthier lives.  

OMH-HDP is interested in learning what your understanding, experience, and interest is about health literacy. We appreciate you taking the time to complete this brief survey. Your responses are voluntary and will be kept confidential. Participant responses will provide an awareness of current health literacy resources and will inform future initiatives. Obtaining your feedback is important to this process! A summary of the results will be shared with the Department in the coming months.  We look forward to hearing from you!
