2010-2011 Dog Bite Statistics Collection Survey
 

Instruction Page

This survey is being sponsored and endorsed by the Coalition for Living Safely with Dogs (CLSD) and the Colorado Association of Animal Control Officers (CAACO) and funded by the Animal Assistance Foundation (AAF).

The goal of the survey is to collect Dog Bite Data on a state wide basis so our organizations can help policy makers in cities and counties make more informed decisions about important animal welfare issues regarding public safety and animals.

Instructions:

1. Please answer as many questions as possible. The more complete data that is entered will ensure a better and more specific study.

2. Please enter only one survey per bite incident.

3. Questions with an asterick * require a response.

4. To avoid duplication of data, a case number, incident number or animal id# is required.

5. A bite or incident date is required.

6. If you answer Yes to Question 6, please complete both the Primary and Secondary Dog Breeds in Questions 7 and 8.

7. If you answer No or Unknown to Question 6, please complete the Primary Dog Breed in Question 7 and select None in Question 8.

8. Please enter the Bite Circumstance in Question 14 that most closely matches the selection in the Drop Down Menu. If the circumstance does not match please select Other and enter the circumstance in the comment section.

9. In Question 15 Level of Injury, please choose only one category, Minor, Moderate or Severe. Please select the choice that best describes the more severe injury. Example. Minor injury selected. The victim may have both a Tooth Scratch and Puncture Wound. Puncture Wound would be selected as the more serious injury.

10. In Question 16 select all contact situations that apply.

11. If you have additional information, please enter it in Question 20. A response to this question is not required but will be helpful in interpreting owner demographics and dog bite behavior.

12. You may change information at any time during the survey. For security reasons once you click the DONE button you may not return to the survey. You will need to contact the Survey Administrator to change information previously entered.
