The Hearing Association Christchurch Inc in conjunction with the National Foundation for the Deaf (NFD) is interested in how you coped both during and after the Canterbury Earthquake in terms of your Hearing Loss or Deafness.

We would appreciate it if you could take time to complete the following questionnaire about your experiences.

The data collected will be useful in determining helpful strategies to ensure that those who have a Hearing Loss or who are Deaf are not disadvantaged in a natural disaster.

Please complete this questionnaire only if:

1. You have a Hearing Loss or are Deaf. OR

2. You are the carer for someone (e.g. a child or elderly person) who has a Hearing Loss or is Deaf
