Healthy Community Institute Pre-Assessment Concord 2012

The Pre-Assessment serves as a benchmark for your nonprofit's participation in HCI and will help to show progress one year later when you will complete a post-assessment. The information from this pre-assessment also helps the faculty tailor the material to meet your organization where you are at.

Responding to this survey is the second step in the registration process and is required in order to confirm your participation in HCI.
