1. HACC Respiratory Therapist Clinical Program Application for Fall 2012

 
By completing this form you are applying for the Fall 2012 Respiratory Therapist clinical program. If you have any questions regarding your application, please contact Professor Leidich - baleidic@hacc.edu - prior to May 1.

All applicants will be notified of the committee's decision after June 1.

2. What is your HACC ID?

3. Have you met (had a one-on-one advising appointment) with Professor Leidich, the Respiratory Therapist Program Director? If not, you are not eligible to apply.

4. Is your program GPA 2.5 or higher? If not, you are not eligible to apply.

5. Have you completed a shadowing tour arranged by Prof. Aumiller within the last 24 months? If not, you are not eligible to apply.

6. Check the courses you have completed or will complete before June 1, 2012 with a C or better:

7. When have you or will you complete Respiratory 100? The course must be taken within 12 months of clinical start.

8. When have you or will you complete Respiratory 110? The course must be taken within 12 months of clinical start.

If you have answered NO to any of the above questions, you may not be ready to apply at this time. If qualified, your application will be presented to the review committee.

9. Do you have a bachelor's degree from a regionally accredited college or university?

10. Have you ever been admitted to (and not completed) another health career clinical program at HACC or another college?
