GP practice survey
We are proud of the service we provide as a GP practice. We are also aware, however, that there will be parts of our services which could be improved. As our patients, you are best placed to let us know how we are doing.
Over the next 18 months, GPs and practice staff are going to become more involved in the planning and designing of all health service in the city. This is part of the Government's NHS reforms. We also, therefore, need to find out from patients their views about other health services such as hospitals.
In view of all of this, we would be grateful if you could complete the following survey. The first part of the survey asks about how you find your GP practice. The second part asks broader questions about other health services you may use. The survey may appear long but it should only take 5 minutes or so to complete.
Please also consider leaving your contact details with us in question 23. There will be more occasions in the future when we want to hear your views on local health services and it would really help us if we could contact you directly.
