A research team consisting of people from the Canadian Treatment Action Council (CTAC) is conducting an environmental scan of care and treatment supports for young people living with HIV (YPLHIV) in Canada.

In order to examine barriers to accessing HIV treatment and care we invite youth-serving community-based and/or Aids Service organizations across Canada to complete this survey. This environmental scan will help CTAC and its community, clinical, research partners, youth-led and youth serving organizations to improve access to care and treatment for YPLHIV. It is about making sexual health services better for youth living with HIV in Canada.

This evaluation survey has been developed with permission from Planned Parenthood, using some questions from Toronto's Teen Sexual Health Survey. The questions are directly related to the objectives for the project. The survey should not take more than about 15 minutes to complete.

Please assist us by including comments in the spaces provided. If you give any question a low score, please tell us how we can suggest improvements in this area. Please also feel free to add in positive comments so we can use these quotes in our evaluation report and celebrate our achievements.
