The following team will be participating in the Indiana M.A.T.H. Bowl on March 1, 2012.

Each participating school must enter the names of their students and coaches, and electronically submit this form by midnight February 19, 2012. Names will appear exactly the way you type them, including capitalization.

Each Elementary Division school may only certify up to 20 students. A team may compete with as few members as you choose. Each team will be composed of students from grades 4-6 with no prescribed number from each grade level.

After you have complete the applicable information for each student, you must click on the CONTINUE button at the bottom of the page. You may then enter the names of your team coaches. After entering your team coach names, click on the DONE button. You will not receive additional confirmation that your form has been received. You will have the opportunity to review/edit the information submitted after entering coaches names, or by using the “Previous” buttons. After submitting the form, it may be possible to return to the form to make additional changes or you can resubmit the entire form, depending on your personal computer settings.

Submitting this form indicates your agreement to abide by the established rules and guidelines, accept the decisions of the Procedural Committee, and adhere to the Code of Conduct and Dress Code.
