The Institute for Johns Hopkins Nursing is pleased to invite you to apply for a full scholarship for the online Guided Care Nursing Course, made possible through a grant from the John A. Hartford Foundation. For more information about Guided Care, please visit www.GuidedCare.org
The scholarship covers the full tuition for our six week, forty hour online course and the American Nurses Credentialing Center’s examination leading to the Certificate on Guided Care Nursing.
Registered Nurses are eligible to apply if they are working for an organization in the United States that has strong interest in using Guided Care in their medical home or ACO (accountable care organization*) model. Priority will be given to RNs working for organizations that have adopted or plan to adopt the Guided Care Model. Although most Guided Care Nurses work with older adults, this is not a requirement for the scholarship.
To apply, complete the requested information on the following pages. You will also need to obtain a signed letter of support from your organization and submit it to us. Instructions for submitting by mail, email or fax are provided at the end of this application.
