Annual Report Survey

Every year, Ontario Shores Centre for Mental Health Sciences (Ontario Shores) produces and distributes an annual report. We are interested in receiving feedback to ensure we are meeting the needs of our various stakeholders. The survey will take less than three minutes. Thank you in advance for your participation.
