PLAY Coach Feedback

1. Feedback

The purpose of this survey is to measure our progress as we continue to strive towards our Positive Coaching goals. This survey is meant to measure our progress and honest feedback is necessary for our volunteer organization to be able to improve.

As always, if you have urgent concerns about any volunteer in our organization, please discuss the matter directly with the individual or escalate to a grade level coordinator or sport director/VP as appropriate.

PLAY is commited to Positive Coaching ideals. We expect Coaches to be "Double-Goal Coaches" who strive to win and to help players learn lessons that will help them to be successful in life outside of sports.

The areas of Positive Coaching evaluation are:
Honors the Game - by showing respect for the Rules, Opponents, Officials, Teammates, and one's Self (ROOTS).

Redefines "Winner" - by emphasizing effort, learning and improvement, and rebounding from mistakes rather than fearing them.

Fills "Emotional Tanks" - with positive encouragement so players can play their best.

Score the questions as follows:

3= Neither Agree or Disagree
5= Strongly Agree
