Sensory History for Adults and Adolescents (Research Version)

 

Dear Parent, Friend, or Colleague:

We have come to realize that to better understand SPD and its impact on individuals, we need to gather more information on typical sensory processing in the population as a whole. By completing an anonymous online survey you will help us gain an increased knowledge of sensory processing in typical adults of all ages and in adolescents aged 13 years and older.

We need as many adults and adolescents as possible from around the country and of all ages to give us feedback. We would appreciate it if you could take approximately 20 to 25 minutes to complete this online survey. Please invite any friends or relatives aged 13 years and older who might also be willing to assist us, to also complete the survey. We hope to have over 2,000 responses within the month!!

If you would lke to print your responses then before cicking "next" at the bottom of each page use your browsers File > Print menu option. If the check marks next to the selected answers are not visible on the printed copy, then adjust the print setting in the browser to show background colors. Each page must be printed individually as you move through the survey.
