The following survey has been designed to get a glimpse of feedback practice in the New Zealand workplace. It has four sections:

1. Thoughts about workplace feedback.
2. Feedback preferences.
3. Receiving feedback: personal experience.
4. Some information about you.

The survey is being conducted by Lines of Sight Limited, a small Wellington-based organisation focused on leadership, team and organisational effectiveness. Lines of Sight has a particular interest in the nature and quality of feedback practice in New Zealand organisations.

If you agree to participate in this survey it will take you 15 - 20 minutes to complete. It could take a little longer if you add comments and it would be great if you had the time to do this.

Information specific to you will remain confidential but you contribution will be used to inform the overall survey results.
