This survey is being conducted by the National Health Resource Center on Domestic Violence (HRC), a project of the Family Violence Prevention Fund (FVPF). The HRC is funded by the U.S. Department of Health and Human Service and was developed to strengthen the health care response to domestic violence by providing free training material and technical assistance.

Your answers to this questionnaire will help us provide materials and improve the delivery of technical assistance.
