FUS Youth Group

FUS youth and adults are looking to reenergize our Youth Group and need your help to ensure that it is fun, interesting and meaningful to you and the other teens that are or have been active in our congregation. Please take few minutes to answer this short survey so we can plan a great year.
First, please provide a little information about you.
1. Are you?
2. What grade will you be starting this fall?
3. How long have you/your family been coming to FUS?
4. What time do/did you primarily go to church?
Give us your thoughts about the Religious Education (“Sunday School”) classes you attended over the years (not including any past participation you may have had in Youth Group).
5. How many years did you regularly attend religious education (RE) classes?
6. For most classes, how frequently did you attend RE classes during the school year?
7. For most classes, how interested were you in going to RE classes?
8. In general, what was the best part of the RE classes you attended? (Check all that apply.)
9. In general, what was the worst part? (Check all that apply.)
10. Did you participate in Coming of Age?
OK, please give us your thoughts about your past experiences with Youth Group.
11. On average, how many Youth Group meetings have you attended each year?
12. If you’ve gone to one or more Youth Group meetings/events in the past, what did you like best (choose all that apply)?
13. What did you like least (choose all that apply)?
Finally, please give us your thoughts about what you would like to see with Youth Group in the future. Although attendance will never be mandatory, we’d like to make it as interesting and enjoyable as possible so people will want to attend regularly and keep it strong.
14. Which kinds of events for Youth Group members would you most likely participate in? (Check all that apply.)
15. Which kinds of events would you most likely participate in that were open to all teens in the community (i.e. church members and non-church members)? (Check all that apply.)

20. What kind of “icebreaker” events do you think would work best to help you get to know other Youth Group members better? (Check all that apply.)
21. Are there past RE teachers/facilitators or church members that you think would make excellent Youth Group advisors? If so, please list their names. If you can’t remember their names, please indicate which class they taught and how many years ago or anything else you can remember that would help us identify them.
22. How likely are you to attend a Youth Group meeting or event next fall with 1 being highly unlikely and 5 being highly likely?
23. Is there anything else we should know to create a Youth program that would keep you interested and engaged?
24. Would you be interested in taking on a leadership role by joining the Youth Group Ministry Team (no ministerial experience required)?
For Question 24, if you answered "yes" or you would like more information, please separately email Janet Swanson, Youth Coordinator, at janets@fusmadison.org
