1. Statement of Informed Consent

Purpose of the Study:
This short survey is part of a study that aims to find out how the library’s website is used and which features are most important.

What will be done:
You will complete a survey, which will take 5-10 minutes. The survey includes questions about how you use the library website and which features are important to you. It also includes questions about which social media platforms you use.

Risks or discomforts:
No risks or discomforts are anticipated from taking part in this study. If you feel uncomfortable
with a question, you can skip that question or withdraw from the study altogether.

Confidentiality:
Your responses will be kept completely confidential. No personally identifying information will be collected in the survey. The library might publish the aggregate results of all survey participants in an academic journal or trade publication.

Decision to quit at any time:
Your participation is voluntary; you are free to withdraw your participation from this study at any time. If you do not want to continue, you can decline to return the survey.

How the findings will be used:
The findings of the study will provide insights which will be used during the redesign of the library website. The findings might also be published an academic journal or trade publication to share insights with other libraries.
