Standards

The 11 Medical Home Standards are an important part of the Medical Home Initiative. A group of more than 40 people representing various state agencies, families, hospitals, organizations, health plans, physicians and policy makers were involved in the development of these standards.

Use this survey as a way for your office to familiarize yourselves with these 11 standards and as a way to identify areas that may need improvement. If you answer "no" to several of the questions, your office may need improvement. Please call us for assistance.
