2010 Associate Giving Form

 

Thank you for making an associate gift to The Foundation for Shawnee Mission Medical Center and/or the United Way. We appreciate your generosity. The Foundation supports numerous programs provided by SMMC especially in outreach to the community. The United Way encourages the community to live, advocate and volunteer.

This on-line giving form is designed to help you make a gift by multiple payroll deductions, one-time payroll deduction, check or credit card.

Associates who gave through multiple payroll deductions in 2009 will continue to have the same amount deducted from their paychecks this year. This means if you are currently giving through multiple payroll deductions you do not need to complete this form unless you are making a change to the gift amount or fund designation.
