This community partner survey is ONLY for 2009-2010 Michigan Service Scholar members - meaning, students who enrolled in the program in the fall of 2009 or winter of 2010. Please clarify with your student to ensure you are filling out the correct form. This survey asks that you provide feedback on students completing service hours with your school/agency/organization.
