5. If your child had ambiguous genitalia, please comment below. Also if your child had any surgeries on his or her genitalia as an infant or child, please explain the problem, the diagnosis, who made the diagnosis and who consented to the surgery or if it was part of a ceremony or religious tradition.

7. If you are expecting a boy soon, have you been counseled by your prenatal provider about the circumcision choice that will be presented to you upon his birth?
