Congratulations on completing your training/internship program. We hope you were able to meet your envisioned goals and objectives outlined in the training plan. We also hope you had a wonderful time here in the US in terms of having cultural experience. To comply with the J-1 visa regulations, you are REQUIRED to complete the End of Stay Evaluation along with your supervisor. The survey should not take more than 10 minutes.
