EnAct Post-Program Survey

 

Thank you for taking EnAct's new online post-program survey. There are opportunities to comment after each section and at the end of the survey, so please share any concerns or suggestions that you have.

You should only complete this form if you have already completed the EnAct program (www.enactwi.org) either as individual or as part of a team.

We will ask questions about your CURRENT behaviors now that EnAct has provided you with tips to live a greener life. This survey is very similar to the pre-program survey you took before beginning the EnAct program. The questions are so similar so that we can measure the changes you have made since completing the program; it should take about 15 minutes of your time.

Please note that you will need the following information on hand:
* current water bill
* current electricity bill
* the percentage of compact fluorescent bulbs (CFLs)currently in your house
* the percentage of low-flow toilets (1.6 gallons or less) and faucets currently in your house

You will also be asked to estimate many aspects of your CURRENT daily behaviors and choices.

While all responses are optional, the information you provide allows EnAct to calculate the impacts of your actions and to show you the results of your efforts.

EnAct does not share or sell any of the information provided to us.
