Adult ESL Practitioner Survey: Perspectives on Diabetes Prevention and Health Literacy Instruction

Page1 / 6

Thank you for taking the time to complete this survey.

Adult ESL instructors throughout the state are being asked to complete this brief questionnaire about their experience teaching health topics, particularly those related to diabetes prevention. The survey results will be used to inform the development of a diabetes prevention curriculum and related professional trainings and implementation plans. We are interested in learning what ESL instructors are already doing that supports diabetes prevention and what additional resources would be needed to improve these prevention efforts.

Note that this survey is part of a research study, and you are not obligated to participate. You are not required to provide your name. Your identifying data will be stored separately from your responses. Only a small number of researchers will have direct access to completed surveys. If this study is published or presented at professional conferences or trainings, actual names and other identifying information will not be used.

If you teach in multiple ESL programs, please answer these questions based on the program you consider your primary teaching context.

If you have any questions, you may ask Maricel Santos, EdD, Assistant Professor, English Department/MA TESOL Program, San Francisco State University; Dean Schillinger, MD, Professor, UC San Francisco School of Medicine, and Chief, California Diabetes Program; Karin Omark, MPH, EdM, Health Promotion Specialist, California Diabetes Program; or the University of California San Francisco Institutional Review Board (UCSF IRB).
