Tuscaloosa County Diabetes Survey

Diabetes: Our Community and You

The Tuscaloosa County Diabetes Coalition's mission is to improve the lives of Tuscaloosa residents at risk or affected by diabetes through community advocacy, education, awareness, and services. To further our mission, the Coalition is conducting a survey to learn more about the needs, interests, and knowledge of diabetes in our community.

The survey will take approximately 5 minutes to complete. Please note the differences between the two most common types of diabetes:

Type 1 Diabetes: Requires multiple daily injections or infusions of insulin. It is sometimes referred to as “juvenile diabetes” or “insulin dependent diabetes”

Type 2 Diabetes: Managed with diet, exercise, oral medication, and/or occasional injections of insulin. It is sometimes referred to as “adult-onset diabetes” or “sugar diabetes”

Thank you for your time and feedback.
