Underwood Survey 2012

 
Dear Patient,

Thank you for agreeing to complete this survey.

We are very thankful to the group of patients who volunteered to help with this small survey. As the group was small, we decided to focus mainly on improving aspects of the surgery relevant to all, e.g. the waiting room. The group also wanted to look at access to the surgery. The doctors wanted to seek your opinion about the availability of Extended Hours appointments.

It would be very helpful to us if you could therefore complete this survey and return it to us. It would be even better to complete it online by going to www.underwoodsurgery.co.uk and clicking on the link on the front page. We will then publish the results in a few weeks.

We are looking for more volunteers for the review meeting when we will discuss how to implement any new changes such as automatic doors, new flooring etc. So please, please if you could spare an hour one early evening in late February or March then do contact our Practice Manager Fred Whalley. In particular we would like to know the opinions of our disabled patients and of younger parents with children.

If this process works well we will undertake more surveys. It would be good to find out our patients' views on for example the repeat prescription service and the system for chronic disease reviews.
