This is a brief questionnaire conducted by Thrive, a trauma-informed system of care, to identify the need for life-skill trainings for youth transitioning to independent living. Questions are designed to incorporate the valuable feedback from youth, guardians, and service providers. The goal is to enable communities to provide the trainings you identify as important.
