Can you help us make a difference?

We invite you to participate in a survey to give FASawareUK information to use as evidence regarding the lack of help and support offered to families/carers. The outcome from this survey will give FASawareUK information to enable us to share with professional organisations. The aim is to help make changes for your child, during their education and their life journey together with its impact on the family and your quality of life.

1. Who officially diagnosed your child with FASD and which hospital/clinician?

2. What age did your child receive an official FAS/D diagnosis?

• Has an early diagnosis and early interventions worked for your child?

• Please give an indication or example?

3. If your child/youth/adult has achieved a FAS/D diagnosis.

How do you feel this affects your child/youth/adult

4. Were you advised when the child was placed with you. That the child may have FASD?
