1. H1N1 Review of Clinic Operations

 
The Connecticut Association of Public Health Nursing (CAPHN) is gathering information to report on the Public Health Nursing Role in CT related to this years H1N1 Response. Please complete these 8 questions related to H1N1 Clinic operations. If you were not involved with an H1N1 Clinic, please respond to the first question and you are done with this survey. A summary of this data will be presented at the CT Annual Local Health Meeting on March 10th and will be used to build a case for public health nursing in CT. PLEASE COMPLETE BEFORE THE CLOSE OF BUSINESS ON FRIDAY MARCH 5th. Thank you for your time and comments.
