The Connecticut Association of Public Health Nursing (CAPHN) is gathering information to report on the Public Health Nursing Role in CT related to this years H1N1 Response. Please complete these 8 questions related to H1N1 Clinic operations. If you were not involved with an H1N1 Clinic, please respond to the first question and you are done with this survey. A summary of this data will be presented at the CT Annual Local Health Meeting on March 10th and will be used to build a case for public health nursing in CT.
